Vancomycin AUC Calculator + Trough Estimator | DrugChug

DrugChug’s Vancomycin Calculator (AUC/MIC + Trough Estimator

DrugChug’s Vancomycin Calculator

DrugChug’s Vancomycin Calculator (AUC/MIC + Trough Estimator) helps students and clinicians-in-training learn the core concepts of vancomycin pharmacokinetics and therapeutic drug monitoring (TDM). This educational tool estimates key PK parameters such as vancomycin clearance (CL), volume of distribution (Vd), elimination rate constant (ke), and half-life, using patient inputs including age, weight, height, gender, and serum creatinine (SCr) to calculate creatinine clearance (Cockcroft–Gault).

To support pharmacology learning, the calculator compares AUC24/MIC and estimated trough concentrations across common vancomycin regimens (for example q12h, q24h, and q36h dosing intervals) and highlights regimens that fall within an educational target range (often discussed as AUC/MIC 400–600). The tool also includes selectable pharmacokinetic approaches such as a Bayesian population estimate, Bauer, and Matzke methods, and it can optionally use one- or two-level vancomycin concentrations to demonstrate how measured drug levels can refine PK estimates (e.g., improving ke and Vd estimation for modeling).

This page is designed for anyone studying antibiotic dosing principles, renal function–based pharmacokinetics, and the relationship between vancomycin troughs, exposure (AUC), and MIC.

Important: This calculator is provided for education and simulation only and does not replace institutional protocols, pharmacist review, or prescriber judgment. Always follow your local guidelines and clinical policies when interpreting vancomycin levels or adjusting therapy.

FAQ

What is vancomycin AUC/MIC? AUC/MIC is a pharmacodynamic exposure metric that compares vancomycin exposure over 24 hours (AUC24) to the organism’s MIC.
What is a vancomycin trough? A trough is the concentration measured near the end of a dosing interval (often just before the next dose), commonly used in TDM education and monitoring workflows.
How is renal function estimated here? This tool uses Cockcroft–Gault creatinine clearance based on age, weight, sex, and serum creatinine (SCr).
